Live Well at Home by Eskaton is the non-medical, Home Care division of Eskaton. Founded in 1968, Eskaton is the largest nonprofit community-based organization serving seniors in the Greater Sacramento area. Our dedicated team members provide services and support for nearly 12,000 individuals.
Live Well at Home’s Sacramento senior care experts bring together an innovative approach to in-home care for seniors utilizing Care Navigators and Non-Medical Home Care to provide a continuum of care not found anywhere else. Live Well’s Transforming Dementia Care allows loved ones with dementia to receive the daily care they need while allowing them to remain in the place where they feel most comfortable and safe – home.
What Do We Do
Live Well at Home offers in-home care for seniors and disabled adults. We assist our clients in daily activities such as:

- Laundry
- Medication Reminders
- Grocery Shopping
- Meal Preparation
- Transportation with Assistance
- Personal Care
- Rehabilitation Care
Our caregivers provide a service which allows our clients to continue living as independently as possible. In addition to caring for our clients, we communicate with family members to help them stay connected to their loved one’s health and safety.
