Transforming Dementia Care
Live Well at Home by Eskaton combines the expertise of our care navigators and dementia care partners to bring your loved one and family an in-depth, person-centered, activities-based memory care program at home.
We’re dedicated to helping your loved one living with the debilitating effects of Alzheimer’s or other dementias to remain at home while enjoying the highest quality of life possible.
At Live Well at Home, we understand that Alzheimer’s and other dementias affect the entire family, not just the individual diagnosed with this disease. Live Well at Home is committed to helping families get exceptional support and dementia care. Additionally, we have integrated the teachings and philosophy of the Eden Alternative®, improving the quality of life for elders living at home and their care partners. Working together, empowered care partner teams, composed of families, care professionals, and the senior, ensures the independence, dignity, and continued growth and development of the whole team by eliminating loneliness, helplessness, and boredom for all.
Our highly-trained dementia care partners have experience with helping seniors and their families deal with the challenges that caring for a loved one with dementia brings about on a daily basis.
We provide transforming dementia care in-home for people with dementia who need a lot of care (normally 4 to 8 hours or more per day) over a longer period of time.
Wellness and Enrichment
Our caregivers can provide services that help your loved one delay the ongoing effects of aging, both mental and physical. We can foster wellness with services that include:
Light exercise program assistance
Continuation of the physical therapy designed by your physical therapist
Companionship and Homemaking
Living longer is not without its challenges; many seniors value their independence and privacy but may eventually require some level of care and assistance. For some, hourly home care may be the only way to allow them to remain in their own home, or to remain at the level of care they are currently receiving in a senior community.
Hourly home care allows you to be in control of setting a schedule for in-home care services that best suit your needs. Whether your loved one would benefit from four hours several days a week or 12-hour shifts, Live Well at Home will work around your schedule. One of our professional care team members will conduct a FREE in-home care assessment to establish the level of care needed and then discuss with you and your family how Live Well at Home can assist in allowing your loved one to remain as independent as possible for as long as possible.
In-home companionship happens where your loved ones are. Whether it is in the comfort of their own home or elsewhere, Live Well at Home companionship and homemaking services will help your loved one maintain his or her independence as long as possible.
Live Well at Home provides companionship assistance for numerous other daily activities, as well as basic peace-of-mind services:

Our respite care services allow you, the caregiver, to “take a break.” Family caregivers need an occasional break. If you do not take time and recharge your batteries, you cannot continue to be effective in your role as caregiver.
Whether it is to take a half day off and go to a movie, take a weekend away, or go on a vacation, Live Well at Home respite care can provide daily, weekly, or as-needed senior care in Folsom consisting of a few hours to overnight or 24-hour care while you are away.
Clients transitioning from a hospital/skilled care center setting back home may require additional support of qualified non-medical caregivers to promote a speedy recovery. Our caregivers are familiar with the needs of clients with mobility restrictions and limitations from post-operation and pain medication side effects. Caregivers work to restore a client’s independence while overseeing and monitoring for safety and risks of injury. Our goals are centered on professional and compassionate care to assist clients with a healthy recovery. Services include:
Picking up prescriptions
Opening up the home
Scheduling follow-up appointments
Hospital to home transitional care services are available 24 hours a day, 7 days a week (including all holidays). Services are available on a short-term, long-term, or routine basis.
