Our at home care solutions are customized to meet your individual needs. We pride ourselves on being flexible. If your care needs change, we can adjust our services and schedule to best meet your needs. We are available when you need us, from a few hours a day up to 24-hour care. We have a long list of at home care services, but the real need is focusing on your wellness and creating a wellness plan unique to you.

From the moment you call Live Well at Home, we begin considering your loved one’s particular needs. During your first call with our office, we will ask questions to help us form a basic needs assessment.

Following the call, we will set up an in-home visit with one of our care navigators and you and the rest of your family – including the family member for whom you are seeking care. This in-person visit allows us to learn more about your loved one’s needs, answer questions you have and help you understand how Live Well at Home can help.

Our Live Well at Home staff has the unique ability to design and implement a comprehensive custom care plan of in-home care solutions in coordination with our care navigators, care coordinators, and care staff.

In-Home Care

Live Well at Home provides private eldercare at home for almost any family and any situation. Our senior care in Folsom allows loved ones to enjoy healthy lives in the comfort of their home. Our caregivers are trained to handle the needs of your loved ones. We will create a custom care plan tailored to your unique situation – providing care from any of our care categories.

Live Well at Home has the unique ability to design and implement a comprehensive custom care plan in coordination with our care navigators and care coordinators.

Care Transitions

Care transitions refers to the coordination and continuity of health care during movement from one healthcare setting to another or to home and between health care practitioners and settings as the client’s condition and care needs change during the course of a chronic or acute illness. Older adults who suffer from a variety of health conditions often need health care services in different settings to meet their many needs. Our care navigators are here to help.

Live Well at Home is uniquely qualified to provide transitional care. Our staff consists of care coordinators and care navigators, who can collaborate in providing a custom care plan to meet your loved one’s needs.

Our care navigators are where the planning begins for your loved one’s care transition. The care navigator can meet with you and your family to discuss the patient’s goals, preferences, and care needs and bring in the appropriate resources to create a smooth transition from a care facility (hospital, skilled facility, etc. to home, whether that be your own home, independent or assisted living.

Some of the care transitions needs may include:
  • Discussion on durable medical equipment needed at home
  • Caregiver training by a physical therapist to continue rehabilitation exercises
  • Transportation from a facility to the patient’s home
  • Pick up prescriptions and medical supplies
  • Checking refrigerator for expired food and shopping for new groceries to meet dietary requirements
  • Arranging follow-up appointments and communication with family members or client representatives
  • In-home care

Our highly-trained dementia care partners have experience with helping seniors and their families deal with the challenges that caring for a loved one with dementia brings about on a daily basis.

We provide transforming dementia care in-home for people with dementia who need a lot of care (normally 4 to 8 hours or more per day) over a longer period of time.

Personal Care

Many times it is during a recovery period at home that your loved one needs a little extra help, or it may be a chronic illness with ongoing needs. We are here to help.

Respite Care

Whether it is to take a half day off and go to a movie, take a weekend away, or go on a vacation, Live Well at Home respite care can provide daily, weekly, or as-needed senior care in Lincoln and the surrounding area consisting of a few hours to overnight or 24-hour care while you are away.

Transitional Care

Clients transitioning from a hospital/skilled care center setting back home may require additional support of qualified non-medical caregivers to promote a speedy recovery. Our caregivers are familiar with the needs of clients with mobility restrictions and limitations from post-operation and pain medication side effects. Caregivers work to restore a client’s independence while overseeing and monitoring for safety and risks of injury. Our goals are centered on professional and compassionate care to assist clients with a healthy recovery. Services include:

  • Transportation home
  • Picking up prescriptions
  • Grocery shopping
  • Opening up the home
  • Scheduling follow-up appointments

Hospital to home transitional care services are available 24 hours a day, 7 days a week (including all holidays). Services are available on a short-term, long-term, or routine basis.
