How We Work

The Live Well at Home difference is found in our people and our process. Our care navigators are able to assess and understand your loved one’s unique situation and recommend a custom home care plan for their particular needs. Our goal is to get your family member the right care as soon as he or she needs it.

The Initial Call and Meeting

From the moment you call our office, we begin considering you and your loved one’s specific needs. On the initial call, we ask questions to form a basic needs assessment.

After the call, we set up a free in-home visit with you and the rest of your family – including the family member who may ultimately benefit from our care. This visit allows us to gather even more information, answer questions, and let you understand how Live Well at Home can help.

The Custom Care Plan

With Live Well at Home, you will always know what in-home care support services are being provided to your loved one. After our in-home visit to get to know you and your loved one better, we develop a custom care plan. This plan is tailored to your loved one’s specific needs.

Care Matching

Once we complete the custom care plan, we begin our search for the most appropriate person to implement care. Our system of matching private in-home caregivers to clients takes numerous factors into consideration: services needed, your loved one’s interests, and the personalities of both your loved one and the caregiver.

Care Supervision

Our caregivers keep extensive notes about the care they provide. Following each shift our Care Pros enter the care notes into their Care Pro portal and they are available to be viewed by every family member set up on the Family Portal. Our billing system also provides a detailed description of services and care so you never receive an invoice that does not include a line-item cost of care. The members of our care team make regular unannounced visits to the homes of our clients to ensure that our caregivers are following the specifics of the custom care plan.

FREE In-Home Care Assessment

An in-home care assessment allows you to sit down with a professional care navigator in your home to gain a better understanding of how our caregivers may assist you. We take this opportunity to learn more about you and what you are looking for in a home care provider. There are six main functions to a home assessment:

  1. A Comprehensive Needs Assessment. We review activities of daily living, cognitive function, and mobility that fit among our list of in-home care services and includes most of the needs we come across.
  2. Preferences and Daily Routines. We help create or uphold daily routines by customizing our services to each individual.
  3. Health and Medication. We assist with doctors’ visits and help with medication reminders. Our caregivers have knowledge, skills and experiences that cover a variety of health conditions common in older adults.
  4. Home Safety Inspection. A home safety assessment is designed to enable seniors to live independently and enjoy their accustomed quality of life without danger of potential injury that could result in a hospitalization.
  1. Cost of Home Care. Every family we serve receives a custom care plan specific to their needs and budget. Our home care services are billed on an hourly basis and the cost is the same hourly rate regardless of the level of care needed. (Shifts over 9 hours are charged an overtime fee.) This makes it easy to make a custom care plan for you. To discuss the exact rates for the services you may need, contact our office. We will set up an in-home consultation to begin discussing a custom care plan that meets your needs. We always provide a copy of our Services Agreement outlining the costs of care prior to initiating care services.
  2. Initial Customized Care Plan. The care plan breaks down the in-home care support services and activities you would expect our caregivers to provide and help facilitate.

Considering Your Options

When selecting an in-home care support services company, you will have many questions. The first thing to understand is that home care services vary from facility-based options. Chances are if you are researching in home care in El Dorado Hills or the surrounding area, you have heard a lot about senior housing options such as  nursing homes, assisted living communities, adult day care centers, retirement communities, and continuing care retirement communities (or CCRCs). These Sacramento elder care options all have unique benefits; however, in-home care is often the preferred choice for seniors who wish to age in place in their own home. Each senior’s financial, social, and health situation will often determine which environment is best.

Depending on your loved one’s needs, you may choose for him or her to receive non-medical home care to provide companionship and supervision throughout the day when you are unable to attend to his or her needs. A non-medical private in-home caregiver is as thoroughly vetted as a caregiver with Registered Nurse or Licensed Nurse Practitioner experience, but may be a better fit for your budget and loved one’s needs.
