As a Live Well Transforming Dementia Care Partner you will provide person-centered, activities-based, dementia care in our clients’ homes—making a real difference in their lives and the lives of their family members—each and every day.

  • Social stimulation (conversation, reminiscing, reading, puzzles, activities)

The most important characteristic we look for in our Live Well Transforming Dementia Care Partners is a heart for caregiving and a positive attitude. You need to be nurturing, compassionate and love helping others. You also need to enjoy spending time with seniors.

  • Experience working with seniors and patients with dementia (family caregiving experience counts)
  • GED, high school diploma, or college degree
  • Must be available to work 8 to 12 hour shifts, including some weekends Many of these clients require 5 to 7 day care – so we prefer care partners with the availability to work multiple shifts per week
  • Speak, understand, and write English fluently
  • Able to initiate activities based on the clients’ needs and in conjunction with their care plan and record client’s engagement with comments on the supplied tablet
  • Ability to work individually and as a member of a team without direct supervision
  • Able to lift 50 pounds
  • Proof of work eligibility
  • Registered as a HCA – Home Care Aide in California
  • Clean driving record
  • Drug free (You must pass a drug test.)
  • Able to pass a FBI and DOJ Live Scan criminal background check
