  • Experienced Dementia Care Navigators

    Live Well’s caregivers are specially trained Transforming Dementia Care partners with advanced dementia training who are hand selected to provide care for clients and who receive ongoing education and support from our care navigators.

  • Customized Activities Based on Client’s Personal History

    In order to create activities which offer the right amount of stimulation and are of interest to the participants, families fill out a detailed questionnaire about their loved one’s history. We then use the person’s hobbies and history, taking into consideration the person’s current abilities, and design meaningful, engaging activities that fit into a regular schedule. We don’t force activities which are too difficult, too childlike, or have no interest to the person. The planned activities include the use of special books written specifically for dementia clients, and custom puzzles can even be created using a client’s family pictures, all done to match a client’s current ability level.

  • Care Plans That Adapt as Needs and Abilities Change

    Regular follow-up visits are performed by a Live Well care navigator to reassess the client’s current level of dementia and make changes to activities according to results. Additionally, quality of life assessments are performed regularly to determine the success of the program and to resolve problems.

  • Creating Solutions for Troublesome Behaviors

    We realize that there are often root causes to problematic behaviors. We attempt to figure out these causes and come up with solutions which address the problems. This can often lead to less need for medication or institutionalization.

  • Creating Meaning and Purpose in Daily Life

    Building a meaningful day involves not only scheduling activities that are essential (like eating, drinking and bathing), and recuperative activities (that allow a client time to rest and recuperate), but also includes scheduling specific activities that focus on wellness, enjoyment and feeling productive or useful. This allows the person to feel as though he or she can still participate in life. These positive feelings contribute to an overall level of satisfaction which reduces behavior problems and leads to an overall happier person.

  • Building Structure Into Each Day

    Many home dementia care plans lack structure and can leave your loved one in front of the TV or sleeping all day. This leads to lower self-esteem and frustration, which often evolves into problematic behaviors such as agitation and wandering. Creating a predictable day with essential tasks and meaningful activities can help your loved one live a happier, fulfilled life.

Live Well at Home is committed to transforming the traditional models of care into person-directed approaches that put the individual first. We have integrated the Eden Alternative® into our Transforming Dementia Care program, and are committed to the creation of vibrant, life-affirming living environments, where everyone has opportunities for growth, regardless of age or abilities.

This program empowers care partner teams, composed of families, care professionals, and the senior to ensure the independence, dignity, and continued growth and development of the whole team by eliminating loneliness, helplessness, and boredom for all. This in-depth Transforming Dementia Care program is supervised by our professional staff of care navigators and care staff.

We Offer Two Levels of Care Under Our Transforming Dementia Care Program

We provide in-home care for people with dementia who need a lot of care (normally 4 to 8 hours or more per day) over a longer period of time. We work to maintain a continuity of care for your loved one and our Live Well Transforming Dementia Care partners.

  • Companion care

    This level of care includes providing companionship and safety, PLUS a person-centered, activities-based, team-focused dementia care plan designed to enrich the senior’s life and make yours a little easier.

  • Personal care

    This level of care includes companion care and also thoughtful, compassionate assistance with dressing, eating, toileting and bathing, PLUS a person-centered, activities-based, team-focused dementia care plan designed to enrich the senior’s life and make yours a little easier.

Additionally, we offer our regular companion and personal home care options for those clients with dementia who do not wish to enroll in the Transforming Dementia Care program.

Family Support

The Live Well integration of the Eden Alternative® philosophy affirms that care is not a one-way street, but rather a collaborative partnership. All caregivers and care receivers are described as “care partners,” each an active participant in the balance of giving and receiving. Together, care partner teams strive to enhance well-being by eliminating the three plagues of loneliness, helplessness, and boredom.

  • Live Well at Home is the only local, licensed home care agency that provides care navigators and dementia trained administrative staff to supervise our Transforming Dementia Care program.

  • Our private in-home caregivers are hand selected for their commitment, compassion and heart for caring for individuals with dementia.

  • Live Well at Home provides community caregiver support groups, some sponsored by the Alzheimer’s Association or the Eskaton Foundation, led by one of our care team members for the families and friends of our clients.

  • We are backed by Eskaton, Northern California’s largest community-based, non-profit serving seniors in our community for over 50 years.

Dementia Care For Senior Communities

Even though your loved one with dementia is in a great community, there are a variety of reasons why he or she might need more one-on-one attention than the community can offer. Live Well’s team of Transforming Dementia Care partners work with clients who currently live in senior care facilities. We offer the following types of person centered dementia care:

  • Immediate Care

    When your loved one with dementia exhibits challenging behavior that threatens his or her status with the place of residence, Live Well can send in a Transforming Dementia Care partner to overcome refusal of care and reset the situation back to a healthier level.

  • Sundowners Care

    People living with dementia often become increasingly agitated during certain periods of the day, most notably during the hours the sun is setting. Live Well can provide a Transforming Dementia Care partner to stay with a dementia patient in his or her community during the times of day of greatest agitation.

  • Transitional Care

    Whether you’re transitioning into or out of a community, Live Well can support that transition by providing a Transforming Dementia Care Partner who is dedicated to ensuring it goes smoothly.
